PROGRAM DESCRIPTION

Service Planning and Assistance Network (SPAN) provides outreach, information and referrals to forensic mentally ill individuals upon their release from New York City courts and jails. Participation in the SPAN program is completely voluntary and is not mandated by the courts.

SPAN’s skilled discharge planning team connects forensic mentally ill individuals to mental health, medical and support services in their communities. SPAN also provides qualified individuals in need of medication with a Medication Grant Program Card, which is accepted at over 3,800 pharmacies in New York State. SPAN offices are located within walking distance of the criminal courts in the boroughs of Manhattan, Brooklyn, Staten Island and the Bronx. A Queens office will open in the near future.

ADMISSION CRITERIA

SPAN provides services to forensic mentally ill adults, 18 years of age or older and reside in New York City. SPAN serves individuals who received mental health treatment while incarcerated in a correctional facility operated by the State, Federal government or New York City Department of Correction.

Individuals incarcerated in correctional facilities operated by the New York City Department of Correction who received treatment for mental illness, and were seen by Mental Health Staff for mental health treatment (2) two times or more, and are diagnosed as seriously and persistently mentally ill (SPMI), will be classified as Brad H. Class Members.

Clients should report to SPAN within (30) thirty days of their release date for comprehensive discharge planning services. Clients that report to SPAN more than 30 days after their release date will receive an assessment, and referrals(s). If the client does not have Medicaid and requires psychotropic medication, SPAN will complete a Medication Grant Program application and submit it for approval. If approved, the client will be given a Medication Grant Program card. To qualify for the Medication Grant Program, the client must report to the SPAN office within seven (7) days of release.

REFERRAL MATERIALS

Criminal justice representatives and community-based service providers can refer clients to SPAN. Clients can also walk-in without a referral for services. Clients must have been released from a New York City court or Jail within the past thirty (30) days, be classified as Brad H. class member (have two (2) mental health treatment visits while incarcerated) or SPMI.

All SPAN offices are located within walking distance of the Criminal Courts in the Bronx, Brooklyn, Manhattan, and Staten Island and is accessible by public transportation.
