PROGRAM DESCRIPTION

The BRC Apartment Program consists of 40 scattered-site apartments in the Fordham Road/Sedgewick Avenue section of the Bronx, and the Upper East Side and the Washington Heights section of Manhattan. The program accommodates 40 residents in individual one-bedroom and large studio apartments. All apartments are accessible to public transportation, places of worship, libraries, parks, colleges and hospitals. The program has established working relationships with hospitals such as North Central, St. Barnabas, Bronx Lebanon, Montifiore Medical Center, the Veterans Administration Hospital, Metropolitan, and Columbia Presbyterian. The program is staffed between 8:30 AM and 7 PM, Monday through Friday and between 10 AM and 6 PM on Saturday.

The majority of tenants are graduates of the Office of Mental Health Certified Community Residence Programs, State Psychiatric Centers and SROs. Referrals from shelters are also accepted. The program accepts tenants who are able to live on their own and have developed the necessary skills to manage their lives within a non – restrictive environment. Individuals with a substance abuse history must have a minimum of one year sobriety.

BRC provides an aggressive outreach to referral sources to ensure that all potential recipients are aware of service and housing availability. The intake process consists of one interview. Upon receiving referral package, those that meet program criteria for acceptance are interviewed and within a week a decision is made. After acceptance into the program, prospective tenants visit three apartments and choose the one that best fits their needs.

Case management services are available to all tenants to assist them in developing skills to maintain their independence. A case manager works with each tenant on quarterly service plans. The case manager also helps the tenant to maintain linkage with entitlement and medical care, enrollment in a day program, vocational training program, ADL and MICA counseling, school, volunteer, and employment referrals.

Support groups and services provided are often located within the tenant’s community, and the Apartment Program staff augments available services.

For example, “Double Trouble” meetings are designed for those tenants who have a mental illness and are also chemical abusers. This program uses the 12 – step recovery process.

Men’s and women’s groups are held regularly in tenants’ apartments. This affords the host the opportunity to show off his/her apartment and to develop socialization skills. The group members decide the topics of discussion.

ADMISSION CRITERIA

The following criteria will qualify a person for placement in BRC’s Bronx Supported Housing Apartment Program:

  • A primary AXIS 1 psychiatric diagnosis
  • Be 21 years of age or older
  • Able to function with minimum support services available according to the demands of the living situation
  • Able to fulfill the financial requirement of the program
  • Must have applied for Section 8 prior to admission
  • Meet the criteria of CSS eligibility as determined by BRC

2) Brooklyn Supported Housing Program

PROGRAM DESCRIPTION

The Brooklyn Supported Housing Program is comprised of 20 scattered – site apartments located in three sections of Brooklyn. Thirteen apartments are located in the East New York section, five apartments are in the Crown Heights section, and the remaining two apartments are in the Flatbush section of Brooklyn. The programs accommodate twenty clients in individual one bedroom or large kitchenette – style units. All of the apartments, with the exception of one, are in elevated buildings, and they are all located in areas that have easy access to public transportation, hospitals, houses of worship, libraries, parks, and academic institutions.

All potential clients have their initial interview at the offices of our Central Intake and Central Entitlements Departments. These first interview verify the level of housing approved, the amount of benefits received, and that all other appropriate and accurate documentation is received. If there is a vacancy, the Program Director and Case Manager of the Brooklyn Supported Housing Program interviews the qualified potential client to ascertain suitability. This second phase of the screening process insures that the criteria of a psychiatric Axis I diagnosis (as delineated in the DSM IV) and at least one year of sobriety (if in recovery from substance use) is met. The program seeks individuals who are able to live on his/her own within a non – restrictive environment.

Once admitted into the Brooklyn Supported Housing Program, all clients are assigned a Case Manager to assist them in formulating, clarifying, and accomplishing goals which the client has outlined he/she wants to accomplish. Our clients are connected to support groups within their community, and participate in monthly Community Meetings. These meetings address issues that may have effect on the entire population, promote peer bonding and support for socialization, celebrate birthdays, anniversaries, and/or personal accomplishments, and provide information pertaining to upcoming events within the agency or outside.

ADMISSION CRITERIA

The following criteria will qualify a person for placement in BRC’s Brooklyn Supported Housing Program:

  • Tenants must have a primary diagnosis of a serious mental illness
  • Tenants must be 21 years of age or older
  • Tenants must be able to function with minimum support services available according to the demands of the living situation
  • Tenants must be able to fulfill the financial requirements of the program
  • Individuals must have a minimum of one year sobriety and demonstrate that they are motivated to maintain sobriety in order to be considered for services

All referral materials should be sent to Centralized Intake. The following documents are needed: psychosocial report, psychiatric assessment, PPD results or chest x – ray, physical report.

3) Metropolitan Apartment Program

PROGRAM DESCRIPTION

The Metropolitan Apartment Program (MAP) is an 87 – bed, scattered – site, transitional community residence program for individuals with psychiatric disabilities and the dually – diagnosed population. It is funded by the NYS Office of Mental Health. The apartments are located in the various neighborhoods of the Upper East Side, Upper West Side, Lower East Side of Manhattan and the North East of the Bronx, where residents share two – bedroom and one – bedroom furnished apartments with 1 or 2 roommates.

MAP
 provides a range of Medicaid – funded restorative services that include medication management, symptom management, money management, daily living activities, community integration, socialization and recreational activities. These services are geared towards assisting the residents to master the necessary living skills to achieve greater independence.

  • A primary AXIS 1 psychiatric diagnosis

4) Stadium Housing

PROGRAM DESCRIPTION

This supportive housing program near Yankee Stadium in the Bronx is designed specifically for mentally-ill, recently homeless and/or chemically addicted adults who have been placed as part of the New York/New York Agreement. Services include socialization skills development, substance abuse counseling and treatment referrals, HIV prevention, pre-vocational counseling, and social and recreational activities. Residents can also participate in the Horizons workforce development program, including paid stipend positions.

REFERRAL MATERIALS

  • Must be single and 18 years of age or older
  • Applicants with a history of drug or alcohol abuse must have at least 6 monhs sobriety
  • Applications are encouraged to be a part of a continued after care program
  • BRC Supported Housing Application
  • Physical exam, including PPD test

Applicants should be capable of living independently–able to conduct activities of daily living such as cleaning, shopping and cooking.
