PROGRAM DESCRIPTION

Clyde Burton House is a 33 – unit supported SRO for older adults who are homeless, housing – needy, and/or living with mental illness. Tenants have their own private room equipped with a bed, a dresser, and a small refrigerator. Two bathrooms and a kitchen area are shared among the seven tenants on each floor. Twenty units are for individuals who are eligible for housing under the terms of the NY/NY Agreement to house homeless individuals with mental illnesses. The other thirteen units are divided between housing needy and community beds.

As a rent-subsidized affordable housing program, tenants must meet certain eligibility criteria and be able to contribute 30% of their income toward rent.

Clyde Burton House provides on – site social services with a social worker and an LPN. Services available to tenants include: case management, group activities, health care monitoring and referrals, medication monitoring, and assistance with activities of daily living.

ADMISSION CRITERIA

The following criteria will qualify a person for placement in BRC’s Clyde Burton House:

  • Applicants must be 55 years of age or older, unless in exceptional circumstances.
  • NY/NY applicants must have an Axis I psychiatric diagnosis and a recent history of homelessness
  • Applicants with a history of drug or alcohol abuse must have at least one year sobriety
  • Applicants should be capable of living independently

  • HRA 1995 Application
  • Psychosocial History
  • Physical Exam, including PPD test
  • Psychiatric Evaluation – if applicable
  • NY/NY Approval letter – if applicable
  • Birth Certificate
  • Immigration Declaration
  • Social Security Card
  • Active Entitlement Documentation (ie: SSI, SSD, or PA)

FUNDING

Services at Clyde Burton House are supported with funds from the New York City Departments of Homeless Services, Health and Mental Hygiene, and Housing Preservation and Development.

Clyde Burton House is located at 330 East 4th Street, between Avenues C and D in Manhattan. 456LNR, Trains to 14th Street – Union Square, then transfer to the M14 – C or M14 – D eastbound to 4th Street
BDFQ Train to Broadway / Lafayette, then transfer to the M21 to Avenue C.
