PROGRAM DESCRIPTION

Blended Case Management (BCM) provides comprehensive case management services to those diagnosed with a serious and persistent mental illness and who often have a history of homelessness and/or substance abuse. In certain instances, the case management teams will need to provide services and monitor clients who are court mandated to participate in treatment under Assisted Outpatient Treatment due to a history of treatment non-compliance.

Case management services include but are not limited to treatment/service coordination, assistance with entitlements, crisis prevention and intervention on a 24-hour a day 7-day a week basis, assistance with daily living skills, assistance with locating and maintaining safe and affordable housing, helping recipients gain access to community resources, and networking with providers, family members, and other community based organizations to provide increased social supports.

ADMISSION CRITERIA

Blended Case Management’s services are specifically targeted towards adults 18 years of age or older who meet the criteria for a serious and persistent mental disorder. Depending on the needs of the client, an ICM (Intensive Case Manager) or SCM (Supportive Case Manager) will be assigned to the client and will meet with them at least twice per month and as clinically appropriate.

REFERRAL MATERIALS

To refer a client to Blended Case Management, please fill out an application (similar to the 1995 Housing Application) and send it along with a recent psychosocial, mental status exam, recent physical and T.B. test/chest x-ray, and a copy of the client’s Medicaid card to Joan Schifaudo.

DIRECTIONS

Blended Case Management is located at 1916 Park Avenue at 131st Street in suites 602 and 605.
Take the 6 train to 125th Street and Lexington Avenue. Walk down Lexington to 131st Street, veer left. The building is across the street at 1916 Park Avenue.
